I have created a new worksheet which can be downloaded and used now.  The aim of the worksheet is to create a handy, wallet-sized wellbeing plan that can be carried with you.  The plan is for you to refer to in difficult moments and an aid to help you communicate you needs and wishes if someone unfamiliar is supporting you. 

The worksheet includes space for noting the things that bring you joy and the things that bring you peace.  It has space for your warning signs when you are finding things difficult and what others can do to support you.  There's also space for you to record your support network, useful numbers, physical health needs, medications and allergies.  Finally on the back pages there is a prompt for whenyou may be feeling overwhelmed and some stress relief ideas.  

This is how the booklet folds up.   Once you have completed your booklet it is suggested you reinforcing the booklet with some clear tape over the pages to protect your notes.  You could also write the date completed on the front page.
